We all want a good death, one where our needs, preferences and wishes are addressed. We understand that many things are important to you and your family at the end of life, and may include:

- To neither hasten nor postpone death
- Your psychosocial and spiritual needs, as well as your families
- To provide support to enable you to live as actively as possible until death

- To have information that you understand, so that you can make good decisions
We offer well planned, coordinated care and support for end of life, that is personal to you and that enables you to remain in familiar surroundings, with close family and friends around you. When planning your care with you and your family, we take into account all the things you tell us that are important to you.
We have a small experienced team, who have the right knowledge, skills and attitude to provide end of life care that is best for you and your life. They are compassionate and appropriate, treating you as an individual, with the dignity and respect that you want. Our dedicated team will support you and your family to prepare for your death. They will work with other healthcare professionals and others involved in your palliative care and home support, to maintain your comfort and choices, with the emphasis being on your quality of life. Our trained palliative care team will help you make informed decisions and plans; using their knowledge and experience, they will explain in plain English anything you do not understand.
